- Neural Tube Defects (NTDs): The most well-documented risk is an increased incidence of NTDs, such as anencephaly and spina bifida. This risk is most pronounced with sustained fevers of 102.2°F (39°C) or higher, particularly if the fever is due to an infectious etiology (flu, UTI).
- Other Congenital Malformations: Studies have also suggested a possible association with other birth defects, including congenital heart defects, cleft palate, and abdominal wall defects.
Second and Third Trimester Risks: Fetal Well-being
While the risk of structural birth defects diminishes after organogenesis is complete, fever in later pregnancy poses other significant threats.

- Assess Your Symptoms: Note any accompanying signs, such as cough, sore throat, painful urination, flank pain, abdominal pain, rash, or severe headache.
- Do Not Panic: While a fever requires attention, it is important to remain calm.

- Tepid Sponging: Use lukewarm water (never cold water, which can cause shivering and actually raise body temperature) to sponge the forehead, armpits, and groin.
- Light Clothing: Wear breathable, light layers to avoid overheating.

- Acetaminophen (Paracetamol/Tylenol): Acetaminophen is the first-line medication for reducing fever and pain during pregnancy. Decades of data have shown it to be safe for use at recommended doses for short durations. It is the preferred agent for managing fever regardless of the trimester.
- Non-Steroidal Anti-Inflammatory Drugs (NSAIDs): This class of drugs, which includes ibuprofen (Advil, Motrin) and naproxen (Aleve), is generally not recommended, especially during the third trimester. Their use in the third trimester can cause premature closure of a fetal blood vessel (the ductus arteriosus), lead to low amniotic fluid, and may complicate labor.
